About Us

Assisted Transition was launched in 2008 offering COMPLIMENTARY resources and placement services for seniors in search of senior care and housing options.  Since then Assisted Transition has helped thousands of families locate the best senior living options by providing referrals to Assisted Living Facilities, Skilled Nursing Homes, Residential Care Homes, Senior Housing and other industry resources.
 
Our unique focus and mission is to make each placement ‘personal’, and by that we mean, we work side-by-side with each placement throughout the entire process.  We do not email or fax leads to senior communities from a call center, instead our local placement owners contact only the communities that are appropriate for the client we are working with.  We do the research.  We do our homework.  We do this because we are local and our advisors live in the area in which they are doing business and attend networking events, participate in fundraisers for local agencies, visit hospitals and Skilled Nursing Homes, and of course, tour appropriate communities and build a relationship on the ground with referral sources and housing options.
 
This unique approach is a great service to the senior and their family.  We currently have over 52 locations across the United States.  We actively partner with local Case Workers, Social Workers, Discharge Planners, Senior Housing Communities, and many other individuals in the Senior Care industry to help them find the best care options for their clients.  We strive to be known as the “Senior Care Concierges” and will actually drive our clients to the communities that fit their needs to tour and guide you through the entire process.
 
Dee Bailey started Assisted Transition – Sacramento out of the desire to do something that mattered. ” I want to work at something that I enjoy and can take pride in and there is no greater satisfaction than helping people through a difficult situation”. Dee has lived in the Sacramento area for over twenty years and has personally visited and interviewed all of the assisted living, residential care, and skilled nursing facilities that she recommends. “Helping people find the right home based on their financial, geographic, and personal preferences is our specialty at Assisted Transition.”
 
 
Dee is a Commissioner on the Sacramento Board of Supervisors Adult  and Aging Commission, Member At Large on the Yolo County Healthy Aging Alliance, Secretary on the Board of the Sacramento Chapter of Case Managers Society of America and Co-Chairman of the Assisted Transition Advisory Council.

I have become a little older since I saw you last, and a few changes have come into my life since then. Frankly, I have become quite a frivolous old gal. I am seeing five gentlemen every day. As soon as I wake up, Will Power helps me get out of bed. Then I go see John. Then Charlie Horse comes along and when he is here he takes a lot of my time and attention. When he leaves Arthur Ritis shows up and stays the rest of the day. He doesn't like to stay in one place very long, so he takes me from joint to joint. After such a busy day I'm really tired and glad to go to bed with Ben Gay.
What a life!

Remember, old folks are worth a fortune - they have silver in their hair, gold in their teeth, stones in their kidneys, lead in their feet, and gas in their stomachs.
